What test markers are included?
HRT Optimization includes the Serum HRT Values Panel.
The blood panel measures:
• Estrone (E1)
• Estradiol (E2)
• Progesterone
• Total Testosterone
• Free Testosterone
• DHEA-S
• SHBG
Together, these values provide a snapshot of key circulating hormone levels while you are using hormone replacement therapy.
Is this covered by insurance or HSA/FSA?
We do not bill insurance directly. Many functional lab tests are HSA/FSA eligible. You can use your card at checkout, though eligibility is ultimately determined by your plan administrator. We recommend checking with your plan administrator before placing your order.
Who is this test best for?
HRT Optimization is designed for women who are currently using HRT and want objective hormone data to share with the healthcare provider managing their therapy.
It may be especially useful if you have recently started HRT, changed your dose or delivery method, continue to experience concerns despite treatment, or simply want to monitor your circulating hormone levels over time.
Because different forms of HRT behave differently in the body, serum testing is more informative for some delivery methods than others. See the HRT considerations below before ordering.
Is serum testing appropriate for my type of HRT?
It depends on the hormone and how it is being delivered.
Serum testing is well suited for:
• Oral estradiol (E2)
• Estradiol patches
• Estradiol pellets
• Transdermal testosterone
• Testosterone injections
• Testosterone pellets
Some forms of HRT require additional context when interpreting serum results:
Estradiol gels and creams: Serum estradiol can rise and fall significantly within a few hours of application, so a single blood draw may underestimate exposure. If you use estradiol gel or cream, In-Depth Hormone Analysis with DUTCH Complete may provide more useful information because it measures hormone metabolites over a longer collection period.
Oral progesterone: Serum testing is generally not recommended for monitoring oral progesterone. Levels can return toward baseline within hours after a bedtime dose, and progesterone metabolites may interfere with standard immunoassay measurements. DUTCH Complete is generally better suited to oral progesterone because it measures urinary progesterone metabolites over the course of the collection day.
Vaginal progesterone: Serum progesterone does increase with vaginal dosing and can provide information about systemic exposure. However, vaginal progesterone also produces high local concentrations in the uterus that are not reflected by serum values, so the blood result does not tell the whole story.
Vaginal estradiol: Standard-dose vaginal estradiol generally produces relatively low circulating estradiol because systemic absorption is intentionally limited. Serum measurement is possible, but symptom response and the reason the therapy was prescribed may provide more meaningful information than the serum value alone.
When should I have my blood drawn in relation to my HRT dose?
The timing of a blood draw can matter because circulating hormone levels may change after a dose, and the appropriate timing can vary depending on the hormone and delivery method.
Follow the collection instructions provided with your laboratory requisition and any timing instructions given by your prescribing provider.
Do not change, skip, or delay your HRT dose solely for testing unless your prescribing provider has instructed you to do so.
Can HRT Optimization tell me whether my HRT dose should be changed?
No. HRT Optimization provides laboratory data and educational information about your results. It does not determine your HRT dose or replace the clinical judgment of the healthcare provider managing your treatment.
Your prescribing provider can consider your laboratory results alongside your symptoms, medical history, current medications, treatment goals, and other relevant health information when deciding whether any change to your HRT is appropriate.
Never modify or discontinue your HRT without first consulting your prescribing provider.
Does HRT Optimization show how my body is metabolizing my hormones?
No. This is an important distinction.
HRT Optimization measures circulating serum hormone levels. It does not measure the downstream metabolites or pathways involved in how your body processes estrogen, progesterone, and androgens.
If you want a deeper look at hormone metabolism in addition to hormone values, In-Depth Hormone Analysis (DUTCH Complete) may be a better fit. For women who want both circulating serum values and a broader look at hormone metabolism, those testing approaches can also be paired.
Where will I have my blood drawn?
HRT Optimization requires a blood draw at a Labcorp location. There is no at-home collection kit.
Your laboratory requisition will be provided through your Practice Better portal after your order is processed.
How long will it take to get my results?
Requisition Delivery: Approximately 1–2 business days after your order is processed.
Lab Processing: Approximately 5–10 business days after your blood draw at Labcorp.
Personalized Lab Results Education Report: Approximately 3 business days after your laboratory results are received.
Is this test available in my state?
The Cycle Clarity Bundle is available to residents of the contiguous United States, with the exception of New York, New Jersey, and Rhode Island due to state regulations.
At this time, testing cannot be ordered if you reside in New York, New Jersey, or Rhode Island, or if you reside outside of the contiguous United States.
What does my personalized lab results education report include?
Your Personalized Lab Results Education Report is designed to turn a complex laboratory report into information that is easier to understand and use. Rather than leaving you to sort through pages of markers, graphs, and reference ranges on your own, your report helps highlight the findings and patterns that deserve your attention.
Your personalized report includes:
- A clear, plain language review of your laboratory findings
- Identification of key results and patterns that stand out
- Education about what your individual markers measure and why they matter
- Connections between related markers and patterns within your results
- Insight into how your findings may relate to the concerns and questions that led you to testing

Your education report is intended to help you better understand your laboratory findings and prepare you for more informed conversations about your health. It does not provide a medical diagnosis or replace care, diagnosis, or treatment from your licensed healthcare provider.
Who is the physician network and what is their role?

The physician network reviews and authorizes your lab order, allowing you to access testing without first obtaining an order from your personal healthcare provider.
The physician network’s role is limited to facilitating and authorizing the laboratory order. They do not provide ongoing medical care, diagnosis, treatment, or replace your relationship with your personal healthcare provider.
Your laboratory results and Personalized Lab Results Education Report are provided to help you better understand your findings and can be shared with your healthcare provider for further evaluation and medical decision making.
