Estradiol gels and creams
Serum estradiol levels can move up and down significantly within a few hours of application, meaning serum testing can easily underestimate actual levels. The DUTCH Complete is better suited for monitoring estradiol gels and creams as it measures hormone metabolites over a longer collection window rather than a single point in time.
Oral progesterone
Serum testing is not recommended for oral progesterone. If taken at bedtime, levels return to baseline within a few hours, making serum results unreliable depending on draw timing. Additionally, progesterone metabolites can cross-react with standard immunoassay tests, producing inaccurate results. The DUTCH Complete is the preferred method for oral progesterone as it measures progesterone metabolites in urine over a full day collection.
Vaginal progesterone
Serum values increase with vaginal progesterone dosing and likely reflect systemic exposure. However, vaginal progesterone benefits from a uterine first pass effect that delivers high concentrations directly to the uterus, which may be the primary therapeutic goal. Serum levels do not reflect uterine hormone concentrations, so clinical interpretation requires additional context beyond serum values alone.
Vaginal estradiol
Serum estradiol levels are generally low with vaginal estradiol use as systemic absorption is minimal at standard doses, which is intentional. Monitoring via serum is possible but symptom response is typically a more meaningful guide than serum values alone for this delivery method.
